We aim to send out all assessment reports within 4 weeks of your final assessment session. The report will be sent to you by email, and you will need to use your date of birth to access it. This document is yours to download and use as you see fit. Your report will also be sent directly to your referral institution.
We will keep a copy of the report and assessment notes securely in your patient record for a period of 8 years as required by Data Protection Legislation, and you can request access to this information at any time by emailing us at ndassessments@problemshared.net.
Depending on the type and outcome of your assessment, either a member of our team or a member of your referrer’s team will be in touch with you to discuss post-diagnostic support.
