Once you have attended a welcome call with a member of our team, you will be invited to download the pre-assessment forms, which include a neurodevelopmental history questionnaire for you as the child's parent/guardian to complete, and a school feedback questionnaire. This must be sent to your child's school for completion by a teacher, SENCo or another member of staff who knows your child well in the school environment.
To access these pre-assessment forms, please log in to your account at www.problemshared.net. From here you will see they appear on your child's client dashboard as available to download, by clicking the ‘Download’ button. Please download, complete and save the form. You can send them back to us by clicking the ‘Upload’ button next to each form on your dashboard and selecting the relevant form to upload.
The School Feedback Questionnaire can be returned to you to upload, or can be emailed to us directly by the school.
If the upload tool is not functioning correctly, or if you have further documents which you believe would be beneficial to your child's clinicians, please send these to ndassessments@problemshared.net where one of the team will upload the files to your record on your behalf.
