In order to ensure a smooth assessment process, the required pre-assessment forms need to be submitted at least 5 working days before the assessment. Unfortunately, we cannot accommodate cancellation or rebooking requests for assessments if the documents are not received on time.
If you are having any issues completing your questionnaires and the answers below do not solve the problem, please contact the support team on ndassessments@problemshared.net.
If you can't edit the questionnaires:
The questionnaires are in editable PDF format, so you should be able to download the form to your device, edit your responses to each question and then save a copy for your own records. Please note that the questionnaires may not be compatibles with certain mobile phones or browsers, so if you are having difficulty editing a questionnaire, please try another device.
If you prefer to hand-write your answers and have access to a printer, we are happy to accept scans or photos of the completed pages via email, so long as your answers are legible.
If your child's school has not completed the School Feedback Questionnaire yet:
As it is best practice to assess for autism across all environments, we do require your child's school to complete our School Feedback Questionnaire. We are aware that schools are busy places, so we make the questionnaires available to you to pass on as soon as you have attended the welcome call. Please send this to your child's teacher, SENCO or another member of staff (i.e. head teacher) who knows your child well and is happy to comment on their experiences at school.
If your child does not attend school currently, or is home-schooled, please advise a member of the team so that the clinicians can take this into account.
If the School Feedback Questionnaire has not been returned by the time the assessment is taking place, the clinicians may not be able to conclude on the day, and therefore a diagnosis will be delayed until the relevant information has been received.
