Where do I send my completed self-report form?

Once you have filled in your/your child's self-report form, you should email it to your GP. You can find the email address of your GP surgery by searching online. In the body of the email, ask your GP to refer you/your child to ProblemShared. You can download our pre-written GP Letter and attach it to the email if you prefer.
