What is my GP Summary Care Record and why do you need it?
Your GP Summary Care Record is an electronic record of your important medical information, created by GP records. This contains useful information around your medical background, allergies or existing medications, which our prescribing clinicians will need to see to ensure that they have all the necessary information to prescribe safely and accurately.
Your GP Summary is required in order for your Treatment Planning Session to be booked, as your clinician will require this information ahead of your session. Only once we receive this information, will your Treatment Planning Session be scheduled.
How do I obtain my GP Summary Care Record?
You can obtain your GP Summary Care Record either from your GP surgery directly or via the NHS App.
Can I download the NHS App for my child, to download their Summary Care Record?
The NHS app is available for clients aged 13 and above, registered with a GP surgery in England or the Isle of Man. Clients under the age of 13 cannot use the NHS App at all. Under the age of 13, parents or guardians can request for their child’s record to be linked to their account. Clients aged 13 to 15 need to contact their practice first and request login credentials for online services before they can access their medical record or order a prescription. The practice may require these patients to undergo a Gillick Competency assessment where they verify the teenager's identity and assess which online services they can access. A client aged 13 to 15 cannot self-register for access to their GP health record. This supports practices that have a blanket ban on 13 to 15-year-olds accessing any online GP services.
I live in Wales, can I still use the NHS App to access my Summary Care Records?
Clients in Wales, will need to download the NHS Wales App. The NHS Wales App is an application specific for patients within Wales. It has been developed and is managed by Digital Health and Care Wales. The NHS Wales App is separate to the NHS App. It does use the NHS login service, however, to enable users to access the NHS Wales App. The NHS Wales App allows clients to access a range of services on their mobile devices or a web browser. Depending on the client's GP practice settings, they can also view their Summary Care Records and full Detailed Coded Records.
How do I upload my GP Summary Care Record?
Your GP Summary Care Record can be uploaded via your dashboard, however if you face any difficulties, this can also be sent via email.
