To make sure we meet the guidelines set by the National Institute for Health and Care Excellence (NICE), we may share your assessment reports with your NHS GP. This could be a full assessment report or a diagnostic letter outlining the diagnosis, key criteria, and recommendations for further care.
If we prescribe medication for you, we’ll also share details of your treatment plan - including your medication and the dose range we expect to adjust during titration. We’ll keep your GP updated on any changes, including, for example, switching medication due to side effects, and once you’re stable, we’ll send a final summary of your care during titration. At that point, we’ll also ask your GP to take over your ongoing care through a shared care agreement.
